Get an X-ray or ultrasound with a hospital-affiliated doctor and you may note a so-called facility fee on your bill ranging from $15 to several hundred or more. The additional charges are showing up on more patients' bills, now that some 40% of doctors work for hospitals. But this year, 15 states are trying to limit these fees.
